My case was approved for medical care

  1. You will receive a claim number and an approved compensable injury. Make certain that Dr. Zegarelli is on your worker’s compensation carrier’s approved list. Your adjustor will be able to give you this information.

  2. Call our office to schedule an appointment. Be sure to have your adjustor’s name and phone number, your claim number, your date of injury, and your approved compensable injury information handy when you call. You will be asked to also give your name, Social Security number, date of birth, employer’s name and address and phone number, as well as other information that must be in your records before an appointment can be made.

  3. Once you have given all the necessary information to your office, we will confirm the approval with your adjustor and schedule your first appointment. We strive to make your first appointment within 1 business day of your call.
